1、本项目的报价方式采用在《采购清单》的单价限价基础上分别报出非挂网产品、挂网产品的折扣率。款项单价结算价格=单价限价×折扣率。2、因一体化格式固化，系统生成《报价一览表》不适用，以招标文件格式《报价一览表（以此为准）》进行报价。

四、招标文件5.4.2.评标细则及标准中“价格分”更正为：

满足招标文件要求且报价最低的供应商报价其价格分为满分，作为基准价。其他供应商的价格分统一按照下列公式计算： 1.非挂网产品投标报价得分=（非挂网产品投标基准价／非挂网产品投标报价）×33×100% 。 2.挂网产品投标报价得分=（挂网产品投标基准价／挂网产品投标报价）×2×100% 。注：产品单项报价不能高于该投标产品在“药品和医用耗材招采管理系统”的挂网最低价格和本招标文件的最高单价限价，超过视为报价无效。（本项目的报价方式采用在《报价一览表（以此为准）》《采购清单》的单价限价基础上分别报出非挂网产品、挂网产品的折扣率。款项单价结算价格=单价限价×折扣率。）
